If you encounter something in the game that we need to take action on, it's important we get all the information we need as quickly and accurately as possible.

Please read the five options below and make sure you choose the most appropriate one for reporting something to us.

1. Do you need to report a concern about a child or adult at risk?

If you have a concern about a child or adult at risk, or someone that is working with children or adults at risk, you can report it by clicking the button below.

You must take action to immediately safeguard children and adults at risk.

You must contact your Club Welfare Officer and if they are not available, or if you are concerned about the immediate safety of someone, contact the Police or Children’s Social Care.

Expert advice can also be provided by the NSPCC Helpline on 0808 800 5000.

2. Do you need to report an incident to us?

Have you been involved in or witnessed an incident that you feel Basketball England needs to be aware of? Have you witnessed racism, discrimination or a breach of the Code of Ethics and Conduct that we need to investigate?

To tell us about an incident, whether you are reporting it yourself or as a witness to something that has happened, please use the button below.

3. Do you want to provide feedback on the performance of an official?

We strive to improving the standards and performance of our officials at all levels of the game. If you were the coach of a game and feel that the performance of the officials was not up to standard, please complete the feedback form and provide as much detail as possible regarding any improvement areas.

• The form can only be completed by the team's coaching staff

• If there is a safeguarding concern, please complete the 'Report A Concern' form above

• Providing their is video evidence, we provide an opportunity for the game to be reviewed by one of our Referee Coaches.

4. Do you need to make a complaint about or to Basketball England?

Are you dissatisfied with the service you have received from Basketball England? Or have you tried to resolve an issue locally, but feel you need to escalate your complaint to us?

If so, please use the button below.

Please be aware of our Complaints Procedure, as well as our Vexatious Complaints and Unacceptable Behaviours Policy, as both will be relevant to making a complaint.

5. Are you an official that needs to report an incident from a game?

This form should be used in the case of a disqualification or other disciplinary matter, or in the case of an official wishing to draw attention to any other matter associated with the match, which may result in disciplinary action.

The contents of this form are confidential and should not be shown or otherwise divulged to any other person.

This must be submitted within 48 hours of the incident or in the case of a Sunday fixture, by 12.00pm on Tuesday.
