If you have a concern about a child or adult at risk, or someone that is working with children or adults at risk, you can report it by clicking the button below.
You must take action to immediately safeguard children and adults at risk.
You must contact your Club Welfare Officer and if they are not available, or if you are concerned about the immediate safety of someone, contact the Police or Childrenās Social Care.

We strive to improving the standards and performance of our officials at all levels of the game. This form should be used in the case of a disqualification or other disciplinary matter, or in the case of an official wishing to draw attention to any other matter associated with the match, which may result in disciplinary action.
The contents of this form are confidential and should not be shown or otherwise divulged to any other person.
This must be submitted within 48 hours of the incident or in the case of a Sunday fixture, by 12.00pm on Tuesday.
