These forms should be used by a person of authority (Coach, Team Manager, Camp Director, Welfare Officer, Therapist, S&C coach) working within the Aspire Programme, England Talent Programme (ETP), England Development Programme (EDP) or Great Britain Age Group teams.
The information submitted will be reviewed during the next working day by the Safeguarding and Integrity Team at Basketball England (BE) and if necessary immediate follow up action taken.
Data will be considered once a month as part of Clinical Governance meetings held by BE. Any significant events will be discussed, analysed and any improvements or lessons learned will be actioned. Relevant feedback will then be provided to appropriate staff members.
All relevant sections of the form required must be completed.
