Accident Reporting Form

Use of this form 

This section should be used for recording accidents, near-misses and sporting injuries on or off the court. All of these will be referred to as accidents in the context of the form.

It is acknowledged that medical staff (i.e. Doctors / Physiotherapists) may have their own systems for recording injuries. However medical staff must still complete this form, and update Medinotes (if they have access) within 24 hours of treating a person.

A near miss is an unexpected event that could have caused harm but didn’t. It is important to report these issues so that we can act to prevent an accident potentially happening in the future. Please also send photos of the area in which the incident occurred in so that we may consider whether it occurred as a result of a defect or fault in either equipment of surroundings.

There are two procedures;

1) Minor accidents

Within our Talent programmes, minor accidents are defined as: "Any accident which results in first aid treatment only and no onward referral."

Management of minor accidents

  • Deal with situation and administer First Aid where required.
  • Make contact with parents / guardians if person is Under 18.
  • Complete the Accident Record for ALL accidents.
  • The Accident Record should be submitted to the Safeguarding and Compliance Team as soon as possible, and within 24 hours of the accident.
  • You should consider, with the player/parent’s consent whether it is appropriate to inform their respective club to ensure ongoing monitoring.
2) Serious accidents

Within our Talent programme, serious accidents are defined as:

  • Any accident which results in a person needing to seek external medical attention
  • Any incident of concussion
  • Any accident which results in attendance at Accident or Emergency Department
  • Any accident which results in hospitalisation
  • Deaths during or within 6 hours of a game or practice session finishing. 

Management of serious accidents

Procedure is the same as for minor accidents above, but you must also:

  • Arrange for the person to seek appropriate medical treatment
  • Contact the BE Talent Manager immediately, so that advice can be sought from BE Medical Team - Sam Messam: 07398 335 094.

Please remember you also have a duty to report all Serious Incidents (such as discrimination online or offline, and abuse against match officials) to [email protected]
