Use of this form 

A significant event is something out of the ordinary which has happened, such as a change to staffing, travel plan or accommodation or something more serious

You should also use this space to complete ‘Neutral Reporting’.  A neutral event is an occurrence that would constitute a breach of policy but where an individual wants to self-report in order to make Basketball England aware that a breach has occurred.  An example of this might be where a member of staff enters a player’s bedroom to attend to an emergency without an accompanying adult.

Please remember you also have a duty to report all Serious Incidents (such as discrimination online or offline, and abuse against match officials) to [email protected]
